Direct3D11 broken after 5.0-3948

Description

What's the problem? Describe what went wrong.

Booting any game with Direct3D11 after 5.0-3948 crashes Dolphin with a "Dolphin.exe has stopped responding" message.

What steps will reproduce the problem?

Leave settings default, change renderer to Direct3D11, try and start a game and crash to desktop.

Which versions of Dolphin did you test on? Does using an older version of Dolphin solve your issue? If yes, which versions of Dolphin used to work?

I had a hunch it was this revision, but I did a bisect anyways.
