There's not enough information in this report for me to accept this.
Regarding 4:3, you have to set that in the Config/Wii menu. Videos may be fixed by using RealXFB, or could be a driver problem, but you didn't put enough information for me to tell.
No thumbs in dolphin interface makes no sense to me, sorry.

Try setting XFB to Real. If that doesn't work, try disabling some other hack settings in Dolphin.

Icons for Wii games will only be displayed if you have a save file for the game.

Does disabling "Store EFB Copies to Texture Only" or "Skip EFB Access from CPU" help? If not, does setting the texture cache accuracy to Safe help?

Dolphin uses a correct aspect ratio automatically, except in games that don't support 16:9. Based on what you've said, this is one of those games. However, if we add a game INI that says that this game doesn't support 16:9, Dolphin will be able to use the correct aspect ratio automatically even for this game. I've done that in https://github.com/dolphin-emu/dolphin/pull/5760 now.

In the past, we had a way to manually add PNG icons, but it currently doesn't work: https://bugs.dolphin-emu.org/issues/10366
