Occasionally Dolphin will fail to initialize the GameCube adapter, the fix is to go into the controller dialog and open the configuration dialog and the adapter will work.
I've been able to semi-reliably recreate this by switching the controller to "Standard" launching a game, then swapping back to the GameCube adapter, but even this is intermittent.
I apologize for being unable to give more detail, but it seems like dolphin isn't even trying to initialize, nothing in the logs indicate an error state or anything like that.
