What's the problem?
DX11 Bug for some games (Metroid other M & Mario Sunshine tested) Metroid other m has big square box of smeared color on left top 1/4 of screen DX11 Plugin only. Mario Sunshine is very distorted same plugin. Other games effected most likely to. R6991 very unstable prone to crash and freezes.
Dolphin version with the problem:
Present in build r6980 - current r6991
(optional) Dolphin version that does not have the problem:
R6974 is fine . (Not tested r6975-6979 not sure there.)
Operating system and version:
32-bit or 64-bit: Win 7 Ultimate X64 OS (X64 Dolphin)
Game ID (as it appears in game properties, Ex.: "GZ2P01" or "RSBE01"):
R3OP01 & GMSP01
Build command-line (not on Windows):
Was the ISO a plain dump from disc, compressed and/or scrubbed?

I have not experienced anything like this on dx11 with metroid other m (game works perfect for me, except for some cutscenes where every once in a while everything goes black)
I know what that square is, but it didn't happened to me since de r4000 or so
Right now i was testing it on r6994 and still not reproductible

the square rectangle in the top left occurs in dx11 in several games due to a EFB regression.

Same here at the top left there is a large square that lights color reigns in the game and this only happens if I use the DX11 plugin. Runs on Dolphin r7185.
in DX9 and OpenGL, they are nice friendly ter in DX11 it's a big square box to left that will not away.
Have tried everything and get it away but it is not. Please fix this problem-If Win7, 4GB RAM, 1GB of dedicated graphics memory that supports DX11, Intel Core i5 760.

I've tried EFB-RAM but it does not help.

I have the same problem using DX11. If I use DX9, I don't have this issue, but it lags a lot when there are too many effects/objects in the loaded area.

I also get the rectangle in the top left. I have found that switching between windowed and fullscreen mode will get rid of it temporarily.

Well, disabling the hack will fix the issue, but cautscenes will become dark again

Create a proper bug report about the cutscene issue when the projection hack is disabled. This issue serves no purpose because there's no or just confusing information available about the actual issue.
Projection hack causing problems is NOT an issue, not even when it only occurs in D3D11.
