CoreAudio on OS X does not obey DSP volume slider unless it's moved while in-game

Description

Yes, this is pretty trivial, but it's probably easy and quick enough to fix that you guys won't mind.

Regardless of what the volume slider in the DSP settings is set to, playing a game under CoreAudio starts it at 100% volume, and requires you to poke (y'know, click on, with or without moving) the volume slider again to get it to how quiet you want it.

This occurs on HLE and LLE and on both sample rates. OpenAL always obeys the slider properly.

(OS X 10.7.3, Dolphin 3.0-600.)
