What's the problem? Describe what went wrong in few words.
Various places in the game, textures will be replaced by a single color with text. This text is related to items in the game, such as "Hammer", "Golden hammer", "heart container", "torchic"

What steps will reproduce the problem?
Use DirectX11, use copy EFB to texture, play one of the stages from the screenshots, I may not sure if it happens every single time but seemed to be most times when I tried in this version of Dolphin.
By using copy EFB to RAM instead of texture, the problem gets fixed.
Which versions of Dolphin did you test on?
4.0-1546 x64
Does using an older version of Dolphin solve your issue? If yes, which
versions of Dolphin used to work?
It was the first time I noticed the issue in this version, it happens fairly often so shouldn't take too long to notice whether it's there in an older version.
What are your PC specifications? (including, but not limited to: Operating
System, CPU and GPU)
Windows 8 x64 AMD Raedon HD 7900, Intel i5 Ivy Bridge

I've had this happen on Metroid Prime too. Usually it will happen again, even with Safe texture Cache.

The hell? I've never seen this in any game. Can anyone give me an exact way to reproduce this; I haven't been able to do it yet.
It looks like a legitimate report, maybe a fifolog or something would be useful, maybe not.

My save has gone missing. I will try it with SSBB instead.

Good enough for me, it was either going to be some kind of weird, unfixable bug, or a really weird user error/driver problem/crazy ass computer thing.
