What's the problem? Describe what went wrong in few words.
Game worked once, every other time I booted it it froze and crashed dolphin at a black screen.
What did you expect to happen instead?
I expected the game to play normally.
What steps will reproduce the problem?

Dolphin 3.5 and 3.5-367 are old versions of Dolphin that have
known issues and bugs, so don't report issues about them and test the
latest Dolphin version first.
Which versions of Dolphin did you test on?

Does using an older version of Dolphin solve your issue? If yes, which
versions of Dolphin used to work?
Not that I recall. New install.
What are your PC specifications? (including, but not limited to: Operating
System, CPU and GPU)
Windows 7, intel i7 4850. intel Iris pro 5200, 8GB ram

Unable to reproduce. You're going to need to provide a full list of settings, the game's md5 hash and anything else you think could be useful.

Where can I find the md5 hash? The settings are totally default, this is a clean install of Dolphin.
The first time the issue occurred, I uninstalled dolphin, reinstalled it, the game opened fine once, and then this problem occurred again.

After a SLEW of reinstalls, it finally works. I have no clue what the issue is. Sorry for wasting time.

I was able to reproduce this yesterday on 2 separate laptops. Both running windows 8.1, one 3630QM + 740m and the other 4700HQ + 840m. Dolphin would work the first boot and after that it wouldn't work anymore. Deleting the "Dolphin Emulator" map with all the settings solved this but then it would only work one time again. Happens on OGL and D3D. The solution is forcing to use nvidia instead of intel in the nvidia drivers. Happens on GC and Wii games.

It happens on every game and not just MKDD. I just noticed it first on that game.

This is some kind of shader cache bug on the Intel HD cards, I'm pretty sure someone else already reported it. If not, then we can retool this one to be that issue.

There is no good issue on this, but I'm closing this one because it's a bit misleading. If you want an issue open with it, more or less make it about OpenGL crashing with a shadercache on IntelHD cards. Thanks!
