What steps will reproduce the problem?
See the cutscenes with Ninja: either after defeating Revolver Ocelot, or after the Ninja fight in order to rescue Otacon.

Which versions of Dolphin did you test on?
Dolphin 4.0-4905

Does using an older version of Dolphin solve your issue?
Didn't try.

What are your PC specifications?
Mac OS X 10.10.1, 3GHz Xeon E5v2, AMD Firepro D700

Fifo 1-frame log: https://yadi.sk/d/SHWKaDcEdmeoG

30-frame log contains some frames before effect is enabled, if you need transient state data.

Looks exactly the same with 1x Native resolution.
EFB2RAM works in both 1x and 4x resolution.

I've found an easier way to reproduce the issue: Just equip Thermal Goggles, effect is the same.
