Qt: Place mapping axis indicators at the top of columns

Description

Because the various mapping columns for Wiimotes have different numbers of options and buttons, the axis indicators are not horizontally aligned which looks messy (see Before screenshot). I made this quick mockup in paint.net of what they would look like with the axis indicators at the top of the columns (and the Calibrate button moved underneath) and I think it looks a lot cleaner. (see After (mockup) screenshot).

In the Extensions tab of the Wiimote mapping pane the Shake section would need to be moved above the Buttons section (which would look more consistent with other mapping windows anyway).

We would need to be consistent and also set the GameCube indicators (including the triggers) to be like this. At first I thought it looked slightly worse this way, but I think I'm just used to the old layout. With some time to get used to it I would probably prefer it this way.

Also, I think showing the actual movement of the joysticks/axes above the various options and mapping buttons makes more sense UI-wise.
