- Game Name and ID (as it appears in right click > properties: "GZ2P01",
"RSBE01", etc): The Last story SLSP01
2) What is the expected output? What do you see instead?
I was fiddling with dolphin settings when I came across this error.
After selecting new game, the game starts loading,
then comes the text:Clearing Code cache
Then a window appears. Warning: GFX FIFO:Unknown Opcode (0x3f, also some others
could be observed)
Second window shows the following information
Warning: Illegal Command 3f (also showed 0x71, probably some others too)
CPBase:0x00c14720 (Stays the same)
CPEnd:0x00c94700 (Stays the same)
CPHiWaterMark:0x0007c000 (Stays the same)
CPLoWaterMark:0x00040000 (Stays the same)

While these windows pop up the audio is on loop
3) Did the game ever work correctly (i.e. not have this problem) on an
earlier version of dolphin? Please specify the exact revision when the
problem began.
The game works in range of 15-30 fps on default settings that come in dolphin
4) What steps will reproduce the problem?
- On ISO properties all Core settings are cross marked. Not Blue boxed.
Wii Console: Enable Progressive Scan is gray, Enable Widescreen is cross
marked.Alternate Wiimote Timing is off. On video only Enable Bounding Box
Calculation is cross marked. - In Dolphin Configuration Enable Dual Core is on, Enable Idle Skipping is on,
Framelimit: Auto, Jit Recompiler, lock threads to cores off, force console
as NTSC-j off. DSP HLE Emulation with Xaudio2, DSP LLE on thread off, dump
audio off. - Graphics.Backend:Direct3D11, Fullscreen res:1920x1080, Aspect Ratio:Auto,
Use fullscreen, show FPS, Internal Res.3x native, AA:none,anistrotopic
filtering 1x, Scaled EFB Copy, Ignore Format Changes, EFB Copies RAM,
Accuracy Fast, External Frame Buffer disable, Fast mipmaps on, disable per-
pixel depth on - Emulated Wiimote: PS2 Game Controller Adapter as Classic extension
5) What version of dolphin are you using (32bit/64bit along with the
version as it appears in the title bar, etc)? Do not say 'latest version'
this changes multiple times a day.
On what operating system, drivers, and hardware? Be sure to list OS,
graphics driver information, and video card model if you are having
graphics problems, for example.

6) Please provide any additional information below.
JITIL experimental recompiler shows the error also.
Both EFB Copies on texture and RAM showed the error.
7) Attachments. IMPORTANT! We have a limited storage quota on
GoogleCode, so please use a 3rd party host for screenshots or any other
files (http://min.us/ for example).

- Status changed from New to Invalid
Turn off the hacks ("Fast mipmaps" and "disable per-pixel"). Also disable "Enable Widescreen".

FYI: Default settings ≠ best settings, by any stretch - otherwise we wouldn't bother giving you the options. If disabling a hack or enabling EFB/XFB fixes something, then it ain't a bug.
Also, the "Other" section of the "Hacks" tab is honestly just a pile of low-benefit options that break things. Every one of those options completely breaks at least one game on my Intel GPU.
