1. Games attempted: Super Mario Sunshine, Metroid Prime, Luigi's Mansion

2) What is the expected output? What do you see instead?
The game should open. In some cases Dolphin crashes, and in other the following error message is given:
"Failed to initialize video back end"

3) Did the game ever work correctly (i.e. not have this problem) on an
earlier version of dolphin? Please specify the exact revision when the
problem began.
I've tested the version in great detail. Here's what I have found:

3.5-119 -> 3.5-134: Crashes when attempting to load any game
3.5-102 -> 3.5-118: Game will not load. Error message "Failed to initialize video back end"
3.5-33 -> 3.5.80 - Crashes when attempting to load game
3.5-3 Works
3.5 - Works

4) What steps will reproduce the problem?

1. Open Dolphin
2. Double click on any game
3. Watch the crash (or error message)

I've attached the crash log from version 3.5-134 (the latest as of this ticket).
