Game Name?
No More Heroes (but I don't think it's limited to just this game)

What went wrong?
The emulated swing does not work.

What did you expect to happen?
The game would register a swing in any of the 4 directions (up, down, left right)

What steps will reproduce the problem?
Go to the first area
Start the tutorial
When you get an enemy low enough, it will prompt you to swing the wiimote to finish them off.
Attempt to swing using either a keyboard or gamepad with keys bound to "swing"

What version of Dolphin were you using (please test the latest version
from http://dolphin-emu.org/download first, 3.5 or 3.5-367 are not valid

I have tried playing with the dead zone, and the range of the swing. Neither does anything.

You'd better download the latest revision from http://dolphin-emu.org/download (currently 3.5-1335) and confirm your bug before a developer close it...

I have tried 3.5-1335 and confirm that it did not improve anything.
I can see that when I use the "swing" in the menu, something is deffinitely happening because the cursor wiggles a tiny bit. But it is functionally broken in game when attempting to use a keybinding that has a swing direction attached to it.

It looks like Issue 5310.
It's a known problem but it's not a priority since you can use a real wiimote.
