V-sync broken on Direct3D 11 and 12

Description

On every build for at least the last six months or so, v-sync has been broken on both Direct3D 11 and 12. The only way to get it working is to force it in Nvidia Control Panel, but that locks the game speed to 100%, with no way to increase it which is inconvenient for RPGs. Trying to start a game with Direct3D 12 selected as the backend will also cause the emulator to randomly crash when the game is launched, especially if you were in the Configuration or Graphics Settings menus. On OpenGL and Vulkan, v-sync is working normally but since I have an Nvidia GPU, there are still stutters with ubershaders.
