What's the problem? Describe what went wrong in few words.
When the game tries to access memory card when looking for saves, Dolphin crashes.

What did you expect to happen instead?
I expect DOlphin will not crash

What steps will reproduce the problem?

1.Run ff4
2.Choose Load from game menu
3.Watch for crash

Dolphin 3.5 and 3.5-367 are old versions of Dolphin that have
known issues and bugs, so don't report issues about them and test the
latest Dolphin version first.
Which versions of Dolphin did you test on?
4.0-4500+

Does using an older version of Dolphin solve your issue? If yes, which
versions of Dolphin used to work?
4.0-3655

What are your PC specifications? (including, but not limited to: Operating
System, CPU and GPU)

By the description of that revision I found that it's because of stereoscopy support for Open GL...
And so, I tested D3D renderer and no crash, but still want to play in GL mode.

I'm using 4.0-4665 and loading a FF4 save game for me doesn't crash Dolphin

Using the latest doesn't automatically mean that intel has fixed all issues, also no memory cards are used for wii games.

New Intel HD drivers + some shadercache fixes should close this issue. Someone correct me if I'm wrong on latest build + latest drivers.
