What's the problem? Describe what went wrong in few words.
When the scan visor is open, there are some lines that form a cross and serve to highlight scannable objects in the environment. Usually these lines are only supposed to be visible within the center scan window. With the bug, they extend to the edge of the screen. In older builds of Dolphin, this happened with fast depth calculation disabled, but it was fine with it enabled. Starting in revision 4.0-6204, the bug happens regardless of the fast depth calculation setting.

What steps will reproduce the problem?
Start the game. When you gain control of Samus, turn on the Scan Visor (D-pad left) and look at any scannable object in the environment.
